Completing the Immunization History Form


International Student Instructions for Completion of Immunization History Form

Students who enroll at the University of Illinois, Urbana-Champaign campus (UIUC) must present proof of immunity against tetanus/diphtheria, measles, mumps and rubella. This includes all newly admitted students, new graduate students, readmitted and transfer students. If you were previously a student at UIUC, please call (217) 333-2702 when you arrive on campus. 

Tuberculosis (TB) Screening Requirements: In order to maintain a healthy environment for students, faculty and staff, the University of Illinois seeks to assure that the campus is free from active tuberculosis. Because different countries have different standards of testing and evaluating for this disease, the University requires that McKinley Health Center, the student health center on campus, screen all incoming international students for tuberculosis. 

Take the following steps to comply with University of Illinois at Urbana-Champaign and State of Illinois requirements:  

  • Ask your physician or nurse to complete the enclosed Immunization History form or attach a copy of physician/school immunization records to the form. 

  • Provide a certified English translation if immunization records are not in English. 

  • Include month, day and year for each immunization (eg: June 15, 2007 would be 06/15/07). 

  • Complete the Medical History form. 

  • Bring the completed forms with you when you travel to the United States. DO NOT MAIL or FAX the forms to UIUC prior to your arrival. 

  • You will receive instructions from the Office of International Student and Scholar Services about submitting these forms to McKinley Health Center upon your arrival. 

  • Keep a copy of these records for your files as part of your personal health record.

  • Tuberculosis skin testing must be performed on campus at McKinley Health Center. You will receive instructions from the Office of International Student and Scholar Services about the required TB screening upon your arrival. 

  • Note: A medical exemption from immunization requirements may be acceptable for: pregnancy or suspected pregnancy, allergies to the vaccine, immunosuppression or severe illness, if you bring a physician’s letter of explanation. 

  • If you are requesting a religious exemption, contact the Office of the Dean of Students at (217) 333-0050. 

Failure to submit the required forms/testing: Missing or incomplete immunization information will prevent you from registering for classes in your second semester as well as changing classes during your first semester.
