International Student Instructions for Completion of Immunization History FormStudents who enroll at the University of Illinois, Urbana-Champaign campus (UIUC) must present proof of immunity against tetanus/diphtheria, measles, mumps and rubella. This includes all newly admitted students, new graduate students, readmitted and transfer students. If you were previously a student at UIUC, please call (217) 333-2702 when you arrive on campus. Tuberculosis (TB) Screening Requirements: In order to maintain a healthy environment for students, faculty and staff, the University of Illinois seeks to assure that the campus is free from active tuberculosis. Because different countries have different standards of testing and evaluating for this disease, the University requires that McKinley Health Center, the student health center on campus, screen all incoming international students for tuberculosis. Take the following steps to comply with University of Illinois at Urbana-Champaign and State of Illinois requirements:
Failure to submit the required forms/testing: Missing or incomplete immunization information will prevent you from registering for classes in your second semester as well as changing classes during your first semester.
